Your child’s scheduled appointment time has been reserved specifically for you. We request 24 hours notice if you need to cancel or reschedule your appointment. There are days when you cannot make your appointment. Please call the office and let us know.
After missing your third scheduled appointment within a 6 month period without notifying the office, we will request to have your child removed from our insurance roster.
Please visit our Forms page to download the forms needed for a New Patient, a Sick Visit or a Vaccine/Physical appointment. You can print the forms before your appointment and fill them out if you would like. Or you can simply review the forms to be sure you have the necessary information in order to complete the forms at our office. We are happy to assist in completing the forms.
Types of Care
Preventative – This is a regular visit to keep you healthy. Call the Doctor.
- Check ups
- Yearly exams
- Immunizations
Sick Visits / Same Day – This is when you may need care right away.
Call the Doctor and make a same day appointment.
- Sore throat
- Fever
- Flu symptoms
- Headache
Emergency – This would be a serious medical condition and are in danger of lasting harm or loss of life if you do not get immediate help. Call 911 or go to the nearest emergency room.
- Poisoning
- Trouble breathing
- Car accident
- Seizures
- Broken bones
After Hours – This is when you need advice or guidance on whether to seek Emergency care or to schedule a Sick Visit. You need to speak to an on call staff member about a symptom your child is having. After regular business hours, on weekends or on holidays, call the office (Phoenix 602-340-9455) and leave your message. Include in the message your name, the patient’s name, the patient’s date of birth and all symptoms. The medical staff on duty will return your call promptly.
