Each visit to Arizona Pediatric Clinics requires an update of your medical records to ensure proper treatment. You can print the forms before your appointment and fill them out if you would like. Or you can simply review the forms to be sure you have the necessary information in order to complete the forms at our office. We are happy to assist in completing the forms.
